Automated Clearing House Recurring AutoPay Payment Agreement


SelfScore, through its bank sponsor, Celtic Bank, a Utah-Chartered Industrial Bank, Member FDIC, has issued the Achieve and Classic Credit Cards. As the information below reflects and based on your consent, through this agreement with you, SelfScore (including “We” or “Us”) and its servicing partner and payments processor, Total Cards, Inc., (TCI) are hereby authorized, directed and empowered to electronically transfer funds from the designated banking account/AutoPay Bank Account for the purpose of making a payment due on the credit card account listed, in the amount specified per the available payment options. Please note that only two payment options are available, as described below. The financial institution you provided, as represented by the bank Routing Number or Debit Card Number, is authorized, directed and empowered to allow such electronic funds transfer to occur. Consent and Agreement for Recurring AutoPay/Preauthorization Payments


Under the federal E-SIGN Act, with your consent, we can deliver information and required disclosures to your electronically and can use electronic records and electronic signatures in connection with your transactions with SelfScore.

By proceeding forward with the AutoPay/Preauthorization Payments process you are providing consent to this payment process regarding payments due for the use of your SelfScore credit card. Further, you agree to receive information relating to this agreement and payment matters electronically. See also Electronic Consent Policy posted on our website.

Recurring AutoPay/Preauthorization Payment Options


You have two payment options available, as described below:

(1) Minimum Payment Amount: Each month, we will debit your AutoPay Bank Account by the “Minimum Payment Amount” referenced in your monthly statement OR the “Statement Balance” whichever is the lower of the two amounts when the AutoPay Payment begins processing.

Note: We will debit your AutoPay Bank Account each month even if you made other payments between the statement issuance date and AutoPay Payment Date, including if you paid the “Minimum Payment Amount”. No debit will be made if you have a zero balance when the AutoPay Payment begins processing. But, remember that if you select this option, you may be charged interest on your account balance.

(2) Monthly Statement Balance: Each month, we will debit the “Statement Balance” referenced in your monthly statement, OR the “Monthly Statement Balance,” whichever is the lower of the two amounts when the AutoPay Payment begins processing. The range of this automatic payment option could be as little as $0.01 up to your total credit line, which can be anywhere from $300 to $5,000 (excluding late fees and interest) depending on whether you have the Classic or Achieve Credit Card; but, please see your monthly statement for your total credit line.

Processing of Payments and Other Information


Accordingly, online payments will be deducted electronically from the banking account specified via Automated Clearing House (ACH), which means that this online payment will generate a single debit entry ACH transaction that will be charged to the banking account specified for this transaction as an ACH debit item. If you have set up automatic and recurring payments, this Agreement covers all such payments scheduled and made.

The monthly credit card statement will serve as notice of payment due on your credit card account. SelfScore/TCI will electronically transfer the Payment Amount from the designated banking account.

Online payments posting information:

Important Payment Posting Information:
  • Payments will be deducted electronically from your banking account via Check by Phone.
  • Our Financial records update each business day. Payments posted to your account will be
    reflected on your account after 6:00 a.m. CST the following business day.
  • One-time payments may not be scheduled more than 30 days in advance
  • Business Day means any weekday (Monday through Friday) except bank holidays.
  • Payments may not result in immediately available credit pending verification of funds.


our monthly billing statement will indicate your “Minimum Payment Amount “ and “Statement Balance” prior to the date on which we will debit your account.

For any automatic payment option that you have selected, SelfScore or TCI will not debit your account by an amount that will cause your credit card account to have a credit balance. You authorize SelfScore and TCI to reduce the amount of the debit previously disclosed on your billing statement so that the amount debited does not exceed the account balance as of the time we begin processing your payment. We will only transfer/access funds in the amounts indicated on your periodic/monthly statement and per AutoPay option selected.

In the event there are insufficient funds in the designated banking account to cover the payment amount, a return payment fee and any applicable late payment fee (as defined in the Cardholder Agreement governing the account) will be applied to the account. You are responsible for paying payment amounts outstanding on your account due to insufficient funds and any return payment fees or late payment fees incurred as a result of insufficient funds.

Other AutoPay/Preauthorized Payments Information


You have the right to stop payment and procedure for doing so. If you have told us in advance to make regular payments out of your account, you can stop any of these payments. Here’s how:

Call us at 1-800-418-7353, or write us at [1010 Doyle Street, Suite 200, Menlo Park, CA 94025, in time for us to receive your request 3 business days or more before the payment is scheduled to be made. If you call, we may also require you to put your request in writing and get it to us within 14 days after you call. (We will charge you [insert amount] for each stop-payment order you give.)

If you believe there was an error in the payment amount debited from your bank account, please call us at the number provided above or write to us.

Notice Of Varying Amounts:
Apart from the payment options stated above, if these regular payments may vary in amount, we will tell you, 10 days before each payment, when it will be made and how much it will be. (You may choose instead to get this notice only when the payment would differ by more than a certain amount from the previous payment, or when the amount would fall outside certain limits that you set.)

Liability For Failure To Stop Payment Of Preauthorized Transfer:
If you order us to stop one of these payments 3 business days or more before the transfer is scheduled, and we do not do so, we will be liable for your losses or damages.

Length of Agreement and Contacting Us


This authorization shall remain in full force and effect until such time as either: (i) we have received written notification from you of its termination in such time and manner as to afford us and the Financial Institution specified a reasonable opportunity to act on it; or (ii) upon the successful completion of the electronic funds transfer agreed to herein.

Definitions


AutoPay Bank Account – This is the bank account you provide at the time of set up, from which your AutoPay Payment will be drafted each month.

AutoPay (Preauthorization) Payment Date – This is your payment due date which will be indicated on your monthly statement.

Minimum Payment Amount – The payment amount required to keep your account in good standing, which will be indicated on your monthly statement. This amount will vary according to your “Statement Balance” each month.

Monthly Statement Balance – Your total outstanding balance as of the date the AutoPay Payment begins processing (includes posted charges and adjustments for payments, credits and/or disputes since the statement issuance date).

Statement (or account) Balance – Your total balance as of the statement issuance date, which will be indicated on your monthly statement.
